Payment Information
Payment of $110 is due in full at the time of the visit. Most insurance companies pay a lesser amount for out of network providers, and my office will provide you with a Superbill to submit to your insurance company for direct reimbursement to you for the covered amount.
*A “no show” or cancellation less than 24 hours before your scheduled appointment time will result in a $75 charge for that appointment*
